A case of Merkel cell carcinoma development under treatment with a Janus kinase inhibitor

چکیده
IL: interleukin JAK: Janus kinase MCC: Merkel cell carcinoma MCPyV: Merkel cell polyomavirus RA: rheumatoid arthritis STAT: signal transducers and activator of transcription INTRODUCTION Merkel cell carcinoma (MCC) is an aggressive neuroendocrine skin cancer thought to be derived from Merkel cells present in the cutaneous basement layer; these cells have synaptic contacts with somatosensory afferents. MCC is a rare malignant tumor, the development of which may be associated with immunosuppressive conditions. Tofacitinib is a Janus kinase (JAK) inhibitor that exerts antiinflammatory activity by inhibiting JAK enzymes, thereby interfering with the JAK signal transducers and activator of transcription (STAT) signaling pathway. Tofacitinib is recommended for individuals with active moderate-to-severe rheumatoid arthritis (RA) that show an inadequate response to traditional disease-modifying antirheumatic drugs or biologic agents. Here we report a case of MCC in an RA patient thought to be caused by immunosuppression during treatment with tofacitinib.

Background: All cellular events depend upon the DNA synthesis and gene expression involving complex interplay between ligands such as interleukins and interferons, with various cell membrane receptors. These ligand-receptors interactions transmit signals within the cell via numerous signal transduction pathways to affect gene expression. Janus kinase/signal transducer and activator of transcrip...

Dispersed neuroendocrine system (D.N.S) consists of a wide variety of cells that are present in the central and peripheral nervous system and in many classic endocrine organs and different tissues such as respiratory and gastrointestinal tracts, skin, prostate, breast and also their neoplasm show neuroendocrine differentiation by electron microscopy, immunohistochemistry or biochemical techniqu...
متن کاملMerkel cell polyomavirus and HPV-17 associated with cutaneous squamous cell carcinoma arising in a patient with melanoma treated with the BRAF inhibitor dabrafenib.
IMPORTANCE Approximately 10% to 25% of patients treated with BRAF inhibitors develop cutaneous squamous cell carcinoma (SCC), but the mechanism responsible has not yet been determined. We report what we believe to be the first case in which Merkel cell polyomavirus (MCPyV) and human papillomavirus subtype 17 (HPV-17) were associated with cutaneous SCC that developed during treatment with the BR...

Sunitinib is an oral tyrosine kinase inhibitor which prevents tumor growth and metastatic progression. It was approved for treatment of advanced renal cell cancer, gastrointestinal stromal tumor and advanced pancreatic neuroendocrine tumors. It has several adverse reactions on multi organ systems including hematologic system. Although the neutropenia and thrombocytopenia commonly happens as Gra...
